The conversion is anchored to ETS's official TOEFL-IELTS linking study: IELTS 7.0 corresponds to TOEFL iBT 94-101, 6.5 to 79-93, 6.0 to 60-78, and so on up to 9.0 ↔ 118-120. CEFR levels follow the IELTS mapping: 8.5+ ≈ C2, 7.0-8.0 ≈ C1, 5.5-6.5 ≈ B2, 4.0-5.0 ≈ B1.
Treat every conversion as a range, not a point: the tests measure differently (IELTS has a live speaking interview; TOEFL is fully computer-based), and universities publish their own equivalence tables that can differ by a few points. When a program lists both scores, meet the one for the test you actually take.
